Creating Medical Homes for Young Adults with Special Healthcare Needs
The PAFP Foundation Medical Home Project
Seventeen family medicine practices from across the state serve as medical home demonstration sites. Those physicians and staff, with the support of the Medical Home Coordinator and an Advisory Council, strengthen their practices' capacity to support adults with special healthcare needs in a medical home.

Training and Other Activities
CME opportunities are available to PAFP members via the Clinical Education Series and stakeholder meetings. Participating practices receive regular visits and training from the project coordinator.

Participating practices also receive resources and tools, such as the new family/patient brochure (PDF, 335KB, 2 pages).

History
Since 2004, the PAFP Foundation has operated the Medical Home Project to enhance the quality and accessibility of healthcare for young adults with special healthcare needs. The project is supported by a grant by the PA Developmental Disabilities Council.
